To save you time on your initial visit to our office, we have provided a form to download and fill out. Completing these forms before you come to our office will allow us to treat your condition more efficiently.

Complete the forms either online or at home and then bring them in when you come to your first appointment.
Your insurance company or managed care plan may require pre-authorization to see one of our specialists. Your appointment may have to be rescheduled if we have not obtained appropriate authorization prior to your office visit.Disability and Work/School Release
Time off from work or school must be established at the time of your office visit. Extensions must be discussed with your physician in advance. We are unable to provide retroactive releases.
Please be informed that it may take several days for our insurance department to complete disability and physical examination forms. The forms are often involved and frequently require chart review, letter comparison and research. Depending on the complexity and time required, a document completion charge may be applied.
Thank you, in advance, for being available when we return phone calls. If your phone has caller ID with anonymous call rejection, please disengage this option if you expect a call from your physician or orthopedic assistant. This will help us make contact with you in a timely manner. We are unable to accept pager numbers for return calls.
Please call your pharmacy for prescription refills. Refills called in to our office by pharmacies are processed between 8:15 a.m. and 5:00 p.m. weekdays. Refill requests are not taken after business hours, on weekends or on holidays. For your protection, refills are not processed for patients that have not been seen in our office within the past two years.
Narcotic prescriptions are usually prescribed for a few days or a few weeks at a time for a specific condition.These medications should be used as directed. Lost narcotic prescriptions will not be replaced.
It is best to become familiar with your insurance plan and coverage. Please refer to the provider lists and other useful information provided to you by your insurance carrier. Some health care services may not be covered under your plan. Our office requires payment of insurance co-payment or co-insurance at the time of service. For your convenience, we accept MasterCard, Visa, American Express and Discover.
Our office is open from 8:30 A.M. to 4:30 P.M Monday through Friday.
